A full periodontal exam: pocket depths at every site, recession measured tooth by tooth, tissue thickness assessed by hand and on intraoral photographs, mobility checked, and a CT scan when the bone underneath needs to be seen clearly. Treatment planning — pinhole vs traditional
Once the diagnosis is in front of us, the technique decision is explained in plain language. Pinhole technique suits select cases where the existing tissue is healthy enough to be repositioned without donor tissue. Traditional grafting — connective-tissue or free-gingival — is the more predictable choice when recession is severe, the tissue is thin, or long-term durability is the priority. The surgery itself
Performed under local anesthesia. Pinhole surgery uses a small entry point to release and reposition existing gum tissue without scalpel incisions or sutures at the surgical site. Traditional grafting harvests donor tissue — most often from the roof of your own mouth, sometimes from a tissue bank when the case calls for it — and secures it with sutures over the recession area. Comfort planning is part of the visit either way.
Early recovery and tissue healing
The first week to ten days are about protecting the graft while the new tissue integrates. Soft diet, careful home care around the surgical sites, and follow-up to confirm healing. Most patients describe the discomfort as manageable rather than severe, especially with pinhole cases. Sutures (where used) are typically removed at one to two weeks.
Long-term maintenance to protect the result
A successful graft is only durable if the conditions that caused the recession are addressed afterward — brushing technique, occlusion, periodontal maintenance, and review of the surrounding tissue. Periodontal patients move onto a recall schedule built around the long horizon, typically every three to four months rather than the standard six.
